For the tour your student will need:

               1. A passport: this takes some time, so starting this process as soon as possible would be advisable.  We will need their passport number to book their flights.

               2. Sign up form: (Different than a release form) The sign-up form has a place for the student to pick their composer and some basic information needed for planning (food allergies, other allergies or medical conditions, medication the student will need to bring with them.)  This form is only seen by Amy.

              3. Release forms: (Medical, Liability, Permission to travel internationally with the group) The medical form will need to be filled out by your physician.  The liability and permission to travel forms must be notarized. The permission to travel forms also include your preferred emergency contacts for your student.  (Luck favors the prepared.) These forms will give us the information and permissions needed in an emergency situation if parents and emergency contacts cannot be reached. While Amy will carry the originals of these forms, the chaperones will also have a copy with them at all times. All forms must be signed by both parents.
